Creates the Health Care Funding Association, a not-for-profit corporation responsible for collecting assessments from health insurers, HMOs, third-party administrators, self-insured employers, and other health care payers to fund immunizations and health care information lines not covered by federal or state funding

Establishes three special funds in the State treasury: the Immunization Program Fund, the Health Care Information Line Fund, and the Health Care Funding Act Administration Fund, with expenditures authorized only by the Director of Public Health

Requires assessed entities to report covered lives and pay quarterly assessments within 45 days after each calendar quarter ends, with assessments calculated on a per-covered-life basis for children (under 19), adults (18-64), and seniors (65+)

Creates a 9-member board of directors including the Directors of Public Health and Insurance, three health carrier representatives, two provider representatives, and one third-party administrator representative, with initial appointments made within 90 days of the Act's effective date

No assessments may be collected before January 1, 2027; late payments accrue interest at 1% per month compounded, and the Association may retain up to 10% of funded costs as an administrative allowance to the Department of Public Health
