Provides services including clinical care, behavioral health, housing, employment, identification restoration, legal benefit access, and care coordination

Targets individuals from 90 days pre-release through 12 months or more post-release from incarceration

Uses closed-loop referrals, shared metrics, and a unified data system to connect participants to services

Effective July 1, 2026
