Creates the End-of-life Care Peer Support Program within the Illinois Department of Corrections for terminally ill incarcerated individuals with conditions likely to cause death within 18 months or those who are medically incapacitated.

Requires the program to operate 24/7 at facilities housing aging or medically vulnerable populations, administered in partnership with hospice organizations, disability organizations, clergy, social workers, and behavioral therapists.

Establishes trained peer support attendants (other incarcerated individuals) to provide non-medical assistance including hygiene help, clerical support, housekeeping, mobility assistance, and facilitated communication with family.

Guarantees participating patients rights to dignity, privacy, culturally competent care, family visitation requests, and access to comfort items such as blankets, memorabilia, music, and books under least restrictive security measures possible.

Funds the program through the Individual Benefit Fund, direct appropriations from the General Revenue Fund, and applicable federal appropriations.
