Establishes the Long-Term Services and Supports Trust Program within the Department on Aging, funded through mandatory employee payroll deductions deposited into a dedicated Trust Fund.

Provides eligible beneficiaries (Illinois residents 18+, not disabled before age 18, meeting assistance needs criteria) with benefit units of up to $100 per day for approved long-term care services.

Covers 17 categories of approved services including adult day services, memory care, respite for family caregivers, home delivered meals, assisted living, nursing home services, and adaptive equipment.

Creates a Long-Term Services and Supports Trust Commission (17 members including legislators, agency directors, providers, and consumers) to oversee program implementation and make policy recommendations beginning January 1, 2027.

Requires biennial actuarial audits starting January 1, 2029 to ensure fund solvency, with the Department of Revenue responsible for collecting premiums and determining individual eligibility.
