Creates the 340B Drug Pricing Program Reporting Act, requiring healthcare entities participating in the federal 340B drug discount program to submit annual reports to the Illinois Department of Public Health starting April 1, 2027.

Mandates covered entities report aggregate drug acquisition costs, payments received, contract pharmacy payments, charity care costs, and how 340B program savings are used, broken down by payer type (Commercial, Medicaid, Medicare, Uninsured).

Requires the Department to compile and publish an aggregated report by November 15 each year, submitted to the General Assembly and posted on the Department's website, while keeping individual entity data confidential.

Allows pharmaceutical manufacturers to request dispensing data from covered entities to investigate duplicate discounts or diversion, including prescription dates, NDC numbers, quantities, and provider identifiers.

Imposes a $1,000 per day penalty on covered entities that fail to submit required information by the deadline; the Act is set to automatically repeal on January 1, 2031.
