Amends the Illinois Public Aid Code to revise site selection criteria for Medicaid supportive living facilities, adding consideration of increasing access for Medicaid enrollees who need alternatives to nursing home care.

Requires the Department of Healthcare and Family Services to conduct a statewide market study report every 3 years (beginning January 1, 2027) to determine geographic need for supportive living program sites.

Defines "need" as a demonstrated deficiency of supportive living program sites for Medicaid-eligible populations age 65 and older, considering occupancy rates, vacancies, waiting lists, and public comments within a 90-day comment period.

Establishes primary market area radius limits varying by location: 4 miles in Cook County, 12 miles in mid-sized counties with populations exceeding 30,000, and 25 miles in all other counties.

Requires the Department to propose implementing rules within 180 days of the act's effective date and sets income qualification at $35,000 or less annually, with provisions for updating based on federal poverty level changes.
