Amends three Illinois Acts: the Assisted Living and Shared Housing Act, the MC/DD Act, and the ID/DD Community Care Act to add the Office of the State Long Term Care Ombudsman to the list of required notification recipients when facilities close.

Facility owners must provide 90 days' notice before voluntary closure or partial closure that would require resident transfers, with notice going to the Department, the Ombudsman, affected residents, their representatives, and family members where practicable.

Notice must include the proposed closing date and reason for closure, and facilities must offer to assist residents in finding alternative placements.

For MC/DD and ID/DD facilities, the 90-day notice requirement applies when partial closures would affect more than 10% of residents.
