Amends the Illinois Identification Card Act and Illinois Vehicle Code to change the third gender option on driver's licenses, permits, and identification cards from "non-binary" to "X" (alongside "male" and "female")

Requires applicants who want to change the sex listed on an existing ID, license, or permit to submit a gender designation form

Specifies the gender designation form may only request: applicant's name, current license/ID number, residential address, gender designation choice ("male," "female," or "X"), attestation statement, and signature

Prohibits the Secretary of State from requiring any additional documentation, certification, or verification beyond the gender designation form to change a gender marker

Requires applicants to attest under penalty of perjury that the gender designation request accurately reflects their gender identity and is not for fraudulent purposes
