Renames the Safety-Net Hospital Health Equity and Access Leadership (HEAL) Grant Program to the "Community Safety-Net Hospital HEAL Grant Program" and narrows eligibility to hospitals meeting safety-net criteria under the Illinois Public Aid Code that are not part of health systems with more than $1 billion in assets

Establishes an annual funding pool of at least $100 million starting in fiscal year 2027, changing from the previous "up to $100 million" language for fiscal years 2024-2029

Requires the Department of Public Health and Department of Healthcare and Family Services to submit a joint report to the General Assembly by February 1, 2027 with recommendations for establishing the permanent grant program

Mandates participating hospitals meet transparency and governance requirements including opening board meetings to the public, having governing boards with at least 51% community members/frontline workers/patients, and creating advocacy councils that meet quarterly

Requires participating hospitals to pay workers a living wage, permit workers to form or join labor organizations without management interference, and maintain adequate staffing with a process for resolving unsafe staffing objections
