Nursing homes and specialized mental health rehabilitation facilities must screen new residents for Social Security benefit eligibility within 30 days of admission using the Social Security Administration's screener tool.

Facilities must assist likely eligible residents with completing Supplemental Security Income (SSI) or Social Security Disability Insurance (SSDI) applications within 60 days of admission.

Within 6 months of the law's effective date, facilities must conduct a one-time assessment of all current residents without income or pending applications and initiate applications for those likely eligible.

Facilities must publicly post on their websites twice per year the number of residents screened and the number of benefit applications initiated.

Amends both the Nursing Home Care Act and the Specialized Mental Health Rehabilitation Act of 2013; takes effect immediately upon becoming law.
