Requires applicants to be Illinois residents and members in good standing of a lawfully chartered Moorish Science Temple, with verification from temple leadership.

Grants the Secretary of State limited power of attorney to correct state, federal, and municipal records to reflect the new name, nationality designation of "Moorish-American," and race classification of "Asiatic."

Mandates the Secretary of State create and maintain a secure database of Moorish Science Temple members in Illinois, updated weekly by 7:00 AM every Monday with new members.

Requires the Secretary of State to implement the process within 60 days, including creating the affidavit template, providing training to employees, and collaborating with designated community members for cultural competence.
