Requires health care professionals to provide mandatory practice information during licensure applications and renewals, including specialty areas, all practice location addresses, hours spent on direct outpatient care at each location, National Provider Identifier (NPI) number, and anticipated retirement date

Mandates the Department of Financial and Professional Regulation to share collected practice information with authorized Department of Public Health personnel

Exempts practice data collected under the new requirements from the Freedom of Information Act and prohibits publishing any data that identifies individual health care professionals

Defines "health care professional" to include those licensed under 13 specific acts covering physicians, nurses, psychologists, optometrists, physical therapists, physician assistants, social workers, nursing home administrators, occupational therapists, podiatrists, respiratory care practitioners, counselors, and speech-language pathologists/audiologists

Requires annual reports by March 1 containing aggregate demographic data, licensure application numbers, and denial counts from the preceding calendar year
