Amends the Environmental Protection Act to require mandatory PFAS (perfluoroalkyl and polyfluoroalkyl substances) testing for facilities with National Pollutant Discharge Elimination System (NPDES) permits designated as "major" facilities by the Illinois EPA and U.S. EPA.

Publicly owned treatment works must conduct periodic sampling of influent, effluent, and biosolids for all PFAS compounds that have accredited wastewater analytical methods available.

All other major facilities must conduct periodic effluent sampling for PFAS compounds with accredited analytical methods.

NPDES permit applications for wastewater discharges with potential PFAS contamination must include full characterization through sample results for all testable PFAS substances.

Permits for land application of sludge or biosolids cannot be issued without PFAS sample results, and issued permits must require periodic PFAS sampling of the applied materials.
