Establishes the Health Care Availability and Access Board, a 5-member independent state body appointed by the Governor to protect residents and the healthcare system from high prescription drug costs

Authorizes the Board to review and set upper payment limits on prescription drugs meeting cost thresholds: brand name drugs/biologics with wholesale acquisition costs of $60,000+ per year or $3,000+ price increases in 12 months, and generic drugs at $100+ for 30-day supply with 200%+ price increases

Requires the Board to adopt federal Medicare Maximum Fair Price as the upper payment limit for applicable drugs, prohibiting the Board from setting different limits than Medicare's negotiated prices

Creates a 15-member Stakeholder Council appointed by legislative leaders and the Governor to provide input on Board decisions, with members having expertise in pharmaceutical markets, clinical practice, and consumer perspectives

Funds the Board through annual assessments on drug manufacturers whose products are sold in Illinois, with enforcement authority granted to the Attorney General; takes effect 180 days after becoming law
