Requires group and individual health insurance plans, managed care plans, and HMOs amended, delivered, issued, or renewed on or after January 1, 2027 to provide medically necessary coverage for peripheral artery disease screening tests for at-risk individuals as defined by American College of Cardiology and American Heart Association guidelines

Extends the peripheral artery disease screening coverage requirement to self-insured counties, municipalities, school districts, and the State Employees Group Insurance Program

Adds the new coverage requirement (Section 356z.80) to the list of mandated health benefits under the Illinois Insurance Code that must be provided by various types of health plans and insurers

Applies the coverage mandate to the Illinois Medicaid program by amending the Illinois Public Aid Code to include peripheral artery disease screening among required health benefits

Amends multiple statutes including the State Employees Group Insurance Act, Counties Code, Illinois Municipal Code, School Code, Health Maintenance Organization Act, Limited Health Service Organization Act, and Voluntary Health Services Plans Act to incorporate the new requirement
