Creates the Office of the State Disability Ombudsperson within the Department of Human Rights to advocate for the rights of persons with disabilities, who represent an estimated 12% of Illinois's population (with another 10% unreported)

Requires the Ombudsperson to offer training, outreach, and educational materials on disability services, rights, laws, benefits, and accessibility by July 1, 2026, and to maintain a toll-free telephone number during business hours

Establishes a complaint process where the Office must receive, review, and act upon complaints regarding violations of disabled persons' rights, and may mediate or advocate on issues including education, employment, health care, housing, and transportation

Requires referral procedures to disability rights organizations, community-based services, and legal advocacy when services are requested or deemed necessary

Mandates annual reports to the Governor and General Assembly by January 15 each year, detailing workload data, complaint types, trends, and recommendations for statutory reform, followed by a formal joint legislative hearing
