Creates the Sexual Orientation, Gender Identity, and Gender Expression in Long-Term Care Bill of Rights Act, establishing protections for residents and home care clients from discrimination based on actual or perceived sex, sexual orientation, gender identity, gender expression, or HIV status

Requires long-term care facilities and home care providers to use clients' chosen names and pronouns, with willful and repeated failure to do so after being informed constituting a violation; clients must be assigned rooms and bathrooms matching their gender identity

Mandates staff training on LGBTQ and HIV cultural competency within 6 months of hire and every 2 years thereafter, with 4 hours required for direct contact employees and 2 hours for non-direct contact employees

Amends the Adult Protective Services Act to include intentional misgendering and unlawful discrimination in the definition of "abuse"

Allows aggrieved persons to enforce violations through civil action and establishes civil penalties and administrative actions for non-compliance
