Requires owners of assisted living, shared housing, MC/DD, and ID/DD community care facilities to notify the Office of the State Long Term Care Ombudsman when closing a facility or any part that requires resident relocation.

Maintains the existing 90-day notice requirement before voluntary closure, with notice also going to the Department, affected residents, their representatives, and family members where practicable.

Applies to partial closures that would require transfer or discharge of more than 10% of residents in MC/DD and ID/DD facilities.

Facilities must continue to assist residents in securing alternative placements and comply with all applicable laws until the closing date.
