Amends the Illinois Domestic Violence Act of 1986 to require law enforcement officers to complete a lethality assessment form when investigating alleged domestic violence incidents

Officers must ask victims 12 standardized questions covering risk factors including weapon use, death threats, strangulation, firearm access, controlling behavior, stalking, and suicidal threats by the alleged abuser

Officers are required to advise victims of assessment results and refer them to local domestic violence programs and the Illinois State Domestic Violence Hotline

When victims cannot provide sufficient information for the assessment, officers must document this in the police report and still attempt to provide referrals

Assessment results and offender identifying information must be reported to supervisors and filed in a manner allowing compilation of aggregate domestic violence data
