Requires health insurance companies to reimburse physicians at minimum 160% of Medicare rates for covered health care services in Cook, DuPage, Lake, and Will Counties

Prohibits physicians from waiving minimum reimbursement protections as a condition of participating in a health benefit plan

Renders any contract or agreement providing reimbursement below the minimum rate void and unenforceable to the extent of the conflict

Authorizes civil penalties up to $15,000 per violation, plus restitution to affected physicians, for insurance companies that violate the requirements

Grants the Illinois Department of Insurance rulemaking and enforcement authority over the new provisions
