Creates a Disability Benefits Maximization Program requiring the Department of Central Management Services and Illinois's 5 state-funded retirement systems to contract with an administrator within 6 months of the effective date.

Mandates annual audits of the State Employees Group Insurance Program, Teachers' Retirement Insurance Program, and College Insurance Program to identify participants and dependents eligible for Medicare but not enrolled.

Requires the program to ensure eligible annuitants, retirees, survivors, and dependents enroll in Medicare as their primary coverage, with state health benefits becoming secondary.

Provides assistance to disabled individuals not enrolled in Social Security Disability Insurance and Medicare, including educational outreach, eligibility analysis, and representation on claims.

Requires annual reports to the Department, the 5 retirement systems, and the General Assembly showing enrollment numbers and state savings realized; effective January 1, 2027.
