Requires individual and group health insurance policies amended, delivered, issued, or renewed on or after January 1, 2028 to cover medically necessary coronary calcium scans and scoring every 36 months for individuals over age 40.

Applies to multiple types of health coverage including state employee group insurance, county and municipal self-insured plans, school district plans, HMOs, limited health service organizations, and voluntary health services plans.

Extends the coronary calcium scan coverage requirement to the Medical Assistance program (Medicaid) under the Illinois Public Aid Code.

Defines "coronary calcium scan and scoring" as a specific medical procedure subject to the coverage mandate.

Takes effect January 1, 2027, with the coverage requirements applying to policies issued or renewed starting January 1, 2028.
