Amends the Illinois Public Aid Code to establish new criteria for selecting supportive living facility program sites, including need for services, funding availability, ability to meet standards, and increasing access for Medicaid enrollees who need alternatives to nursing home care.

Requires the Department of Healthcare and Family Services to conduct and publish a statewide market study report every 3 years beginning January 1, 2027, to determine geographic areas with demonstrated deficiencies in supportive living program sites for Medicaid-eligible populations 65 and older.

Defines "primary market areas" with radius limits varying by location: 4 miles in Cook County, 12 miles in mid-sized counties with populations over 30,000, and 25 miles in all other counties.

Establishes eligibility criteria for the market study based on age cohorts (65-74, 75-84, 85+), self-care difficulty or dementia prevalence rates, and annual income of $35,000 or less, with a 20% capture rate applied to determine eligible population.

Requires the Department to propose implementing rules within 180 days of the Act's effective date and provides a 90-day public comment period following publication of each statewide market study report.
