Illinois residents aged 20+ would be entitled to free heart disease screenings: every 6 years for ages 20-39, and every 2 years for ages 40 and older.

Screenings must include blood tests (cholesterol, triglycerides, blood sugar), blood pressure check, electrocardiogram, echocardiogram, exercise stress test, and coronary calcium scan with no co-payments, deductibles, or cost-sharing.

The State would serve as payer of last resort, covering costs only for uninsured or underinsured individuals whose private insurance, Medicaid, or Medicare does not fully cover screenings, through a voucher program.

The Department of Healthcare and Family Services must establish a confidential HIPAA-compliant registry to track screening dates and prevent duplicate screenings, with an opt-out option for individuals.

Subject to appropriation, the voucher program would be implemented by July 1, 2027, with conforming amendments to the State Employees Group Insurance Act, Counties Code, Illinois Municipal Code, School Code, and Illinois Insurance Code.
