Requires voters to present acceptable photo identification or a voter identification card to election judges before voting in Illinois elections.

Acceptable photo IDs include Illinois driver's license, state ID card, disabled person ID, senior citizen ID, Firearm Owner's ID, U.S. passport with current address, or other government-issued photo ID with name and current address.

Directs the Secretary of State to issue voter identification cards to registered voters who lack acceptable photo ID, with cards containing the voter's name, signature, photograph, State seal, and residence address.

Allows voters who are indigent (household income at or below 125% of federal poverty guidelines) or have religious objections to being photographed to vote by provisional ballot with a signed affidavit.

Establishes documentation requirements for obtaining a voter ID card, including birth certificates, voter registration cards, utility bills, bank statements, or tax returns to verify identity and address.
