Declares Indiana's sovereignty over health care choice under the 9th, 10th, and 14th Amendments to the U.S. Constitution, asserting that health care decisions are intrastate activities exempt from federal regulation under the Commerce Clause

Defines "health care choice" as the freedom of Indiana residents to choose how their health is maintained, including the right to not participate in any health care system or program

Exempts Indiana from federal laws mandating national health insurance, controlling employer health insurance decisions, rationing health care, requiring use of federal databases for patient information, or imposing penalties for non-compliance

Requires health care providers to maintain records proving patients are Indiana residents, inform patients of their right to opt out of federal databases, and annually submit forms to the state department of health affirming their business is wholly located in Indiana

Directs the Attorney General to provide legal defense of this chapter and defend Indiana health care providers and residents against federal government actions attempting to limit health care choice or force compliance with federal health care laws
