Requires nursing facilities to notify applicants of the preadmission screening requirement, potential Medicaid ineligibility for up to one year if screening is not completed, and provide contact information for the local agency and a list of available long-term care options.

Mandates that preadmission screening assessments evaluate the individual's medical needs, availability of non-facility services to maintain them in the least restrictive environment, and cost-effectiveness of alternatives to nursing facility care.

Requires a representative from the local agency familiar with personal care assessment to explain and provide written assessment results to the individual or their guardian as soon as practicable, including information about identified services.

Requires hospitals to provide patients being discharged for preadmission screening with a list of available long-term care options in the hospital's service area, including agency contact information and notification that free counseling is available.

Takes effect July 1, 2011.
