Requires physicians to obtain oral and written informed consent at least two weeks before performing a hysterectomy or oophorectomy

Mandates patients receive specific educational materials including a DVD titled "Female Anatomy: The Functions of the Female Organs" and a pamphlet from the HERS Foundation, plus color diagrams of pelvic organs, supporting structures, nerve supply, and blood supply

Patients must sign a written statement confirming they read and understood the materials, discussed them with their physician, and were advised the procedure will cause permanent sterility

Exempts life-threatening emergency situations where prior written informed consent is not possible

Physicians who fail to comply face disciplinary sanctions through the medical licensing board, effective July 1, 2011
