Prohibits dental insurers from requiring dentists to accept insurer-set payment amounts unless the services are covered under the insured's policy, effective July 1, 2011.

Prohibits health maintenance organizations from requiring dentists to accept HMO-set payment amounts unless services are covered under the enrollee's contract.

Prevents insurers and HMOs from providing only minimal reimbursement or coverage as a workaround to avoid these requirements.

Excludes discount medical card program provider agreements from these requirements.

Establishes violations of these provisions as unfair and deceptive acts under Indiana insurance law.
