Creates "covenant marriage" as a new legal marriage designation in Indiana, defined as a marriage between one male and one female who agree the marriage is a lifelong relationship and that dissolution may only be sought by a non-breaching party when there has been a complete breach of the marital covenant

Requires couples seeking a covenant marriage to complete at least 4 hours of premarital counseling across at least 2 sessions that emphasize the nature, purposes, and responsibilities of marriage

Allows existing married couples to convert their marriage to a covenant marriage by filing a declaration of intent with the clerk of the circuit court, along with attestation of counseling received

Requires the state department of health to maintain separate records distinguishing covenant marriages from standard marriages and to develop uniform forms for covenant marriage declarations

Directs the attorney general to develop an informational pamphlet titled "Covenant Marriage Law" outlining the consequences of entering into a covenant marriage, to be made available to clergy and marriage counselors
