Prohibits accident and sickness insurance policies and health maintenance organization contracts from providing coverage for elective abortions, effective July 1, 2011.

Allows insurers and health maintenance organizations to offer elective abortion coverage only through a separate rider or endorsement with a separate premium paid independently from the base policy.

Requires riders or endorsements for elective abortion coverage to calculate premiums based on actuarial cost estimates, obtain separate signatures, and provide written notice disclosing the specific cost and optional nature of the coverage.

Prohibits insurers or health maintenance organizations from offering premium reductions on base policies to individuals who purchase elective abortion riders or endorsements.

Requires employers and other entities offering elective abortion coverage riders to allow employees or individuals to refuse coverage at initial enrollment and at least annually thereafter.
