Requires petition carriers circulating petitions to place candidates or public questions on the ballot to sign an affirmation on each petition page stating under penalties of perjury that signers are eligible and properly completed the petition.

Mandates petition carriers print their full name, complete address with ZIP code, and date of birth below the affirmation on each petition page.

Voids any signature page that does not comply with these requirements or signed by a petition carrier who fails to meet the specified qualifications and requirements.

Applies to all petitions required to place a candidate or public question on the ballot, unless a specific petition statute provision conflicts with this chapter.

Takes effect July 1, 2012.
