Requires parties to guardianship, custody, or parenting time proceedings to inform the court in writing if any party has been determined to be a perpetrator in a substantiated child abuse or neglect assessment

Mandates disclosure when the child named in a petition has been the subject of a substantiated abuse/neglect report, determined to be a child in need of services under IC 31-34, or involved in an informal adjustment

Applies to petitions establishing or modifying guardianship for minors under 18, child custody, and parenting time (visitation) across multiple Indiana Code sections

Authorizes courts to request Department of Child Services assessment or case information regarding any party or child involved in these proceedings

Requires the Department of Child Services to respond to court information requests under seal within 10 days
