Requires the office of the secretary of family and social services to develop and implement an evidence-based program model partnering with elementary and high schools to provide social services to children, parents, caregivers, teachers, and the community.

Program model must include substance abuse services and prevention, assistance for at-risk students, grief counseling and suicide prevention, parenting skills education, and social skills development.

Allows the office of the secretary to contract with nonprofit organizations that have at least five years of experience providing evidence-based prevention programs and employ at least ten individuals with master's degrees in social work.

Requires the office of the secretary to hire an independent third party to evaluate program outcomes and prepare annual reports measuring the contractor's effectiveness and program impact.

Effective July 1, 2013, with option to initiate the program as a pilot project in southwestern Indiana.
