Prohibits dental insurers and health maintenance organizations (HMOs) from requiring dentists to accept insurer-set payment amounts for services that are not covered under a patient's insurance policy

Defines "covered services" as health care services eligible for any reimbursement, regardless of limitations such as deductibles, copayments, coinsurance, waiting periods, or annual/lifetime maximums

Exempts discount medical card program provider agreements from these requirements

Classifies violations as unfair and deceptive acts in the business of insurance under Indiana Code

Effective July 1, 2013, applying to contracts entered into, renewed, or modified after June 30, 2013
