Amends IC 34-28-2-2.5 to allow courts to waive the requirement that name change petitioners include an Indiana driver's license number or identification card number for good cause shown.

Applies to petitioners at least seventeen years of age who are filing for a name change under Indiana law.

Maintains all other petition requirements including date of birth, residence address, list of previous names, proof of U.S. citizenship, passport information, and criminal conviction history.

Becomes effective July 1, 2013.
