Adds definition of "state immunization registry" and requires health care providers to enter administered immunizations into this registry instead of providing paper certificates.

Eliminates requirement for schools to submit immunization reports to state department of health; instead requires state department to obtain reports by accessing the state immunization registry at the state and aggregate level.

Makes disaggregated staff performance evaluation results reported by school corporations exempt from public record requests under Indiana's public records law.

Changes student count date for special education grants from December 1 to October 1 of the preceding fiscal year.

Repeals requirements for schools to provide information about meningococcal disease and HPV vaccination to students and parents, and eliminates the requirement that school immunization records be kept uniformly throughout Indiana.
