Department of Insurance must develop a uniform prior authorization form for prescription drug coverage by December 1, 2014, not to exceed two pages and available electronically.

Form development must include at least one public hearing with interested parties and consider Medicaid prior authorization forms and national electronic prior authorization standards.

Beginning July 1, 2015, prescribing providers must use the uniform form to request prior authorization, and insurers and health maintenance organizations must accept completed forms as sufficient for determination.

If an insurer or health maintenance organization fails to accept the form or respond within two business days of receiving it, the prior authorization request is automatically deemed granted.

Effective upon passage with an emergency declaration included.
