Amends IC 16-38-5-3 to allow the state department of health to release immunization data registry information to child care ministries registered under IC 12-17.2-6 in which an individual is enrolled.

Adds child care ministries to the list of entities authorized to receive immunization data, alongside existing recipients such as providers, schools, licensed child care centers, and colleges/universities.

Requires all recipients of immunization data to enter into a data use agreement with the state department restricting re-release of patient-identifying information without written consent, unless releasing to authorized entities.

Maintains confidentiality protections for immunization data registry records while permitting release to individuals, parents, and guardians of minors.

Takes effect July 1, 2014.
