Requires a treating physician to evaluate an individual's mental capacity and determine the individual is mentally able to understand executing a POST form before the form may be executed

Mandates that the treating physician and qualified person or representative must sign and date the POST form for it to be effective

Establishes that the first POST form executed by an individual is effective for six months from the date of execution

Allows POST forms to be renewed with no expiration date upon renewal, unless revoked
