Defines "qualified health plan" as a health plan certified under Section 1301 of the Patient Protection and Affordable Care Act to meet criteria for availability through Indiana's health benefit exchange.

Requires the office of the secretary of family and social services to apply for a Medicaid state plan amendment or demonstration waiver before July 1, 2014 to purchase qualified health plan coverage through the Indiana health benefit exchange.

Limits coverage to individuals aged 19-64 with income not exceeding 138% of the federal poverty level who are not eligible for Medicaid in Indiana and not eligible for Medicare.

Allows the secretary to require cost sharing by individuals obtaining and maintaining coverage under the qualified health plan.

Prohibits implementation of the state plan amendment or waiver until the secretary files an affidavit with the governor confirming federal approval, with implementation required within 60 days of governor notification.
