Amends Indiana Code 31-11-4-4 to require the state department of health to develop uniform marriage license application forms that accommodate applicants' preference for either gender-specific or gender-neutral forms.

Requires marriage license application forms to include each applicant's Social Security number, which must be kept confidential and used only for Title IV-D program purposes, with violations constituting a Class A infraction.

Maintains existing application requirements including full name, birthplace, residence, age, dependent children information, parental information, and a statement regarding impediments to marriage.

Requires both applicants to acknowledge receipt of information about sexually transmitted diseases and AIDS test site locations, with exceptions for those with religious objections.

Allows persons objecting on religious grounds to forgo oath/affirmation verification and the disease acknowledgment, with special provisions for Old Amish Mennonite church members requiring bishop certification.

Effective July 1, 2016.
