County fiscal bodies may adopt an ordinance allowing counties, political subdivisions, and local agencies to use a uniform property tax disclosure form for applications related to property tax exemptions, deductions, zoning changes/variances, building permits, and other licenses or permits.

The uniform disclosure form must require applicants to describe all real property parcels they own in the county and provide a verified statement identifying which parcels have current tax liability and which have delinquent tax liability.

Counties, political subdivisions, and local agencies may require applicants to submit the uniform disclosure form with their applications and may require proof of current tax payments for each parcel before approval.

Local entities may establish policies requiring applicants with delinquent tax liabilities to enter into payment agreements with the county treasurer, or may refuse to approve applications until delinquent taxes are paid in full.

The state board of accounts and department of local government finance must provide assistance to county fiscal bodies in prescribing the uniform disclosure form upon request; effective July 1, 2017.
