• Establishes a Medicaid accountable care pilot program to be developed and administered by the office of Medicaid policy and planning, requiring implementation by April 1, 2018.

• Eligible participants are Medicaid recipients in the aged, blind, and disabled category who participate in the fee-for-service program; excludes those in institutional waivers, dually eligible for Medicare/Medicaid, or in risk-based managed care programs.

• Accountable care organizations must include health facilities, developmental disability providers, mental health providers, home health agencies, case management providers, and managed care organizations that share patient information and ensure rapid service transitions.

• Office must develop a shared savings component allowing participating organizations to receive a portion of cost savings from coordinated care and provide organizations with relevant historical and financial data.

• Office must develop success metrics measuring participant health improvement, satisfaction, provider communication, and cost savings through care efficiencies, then publish a report by December 31, 2020; program expires December 31, 2021.
