Allows unregistered Indiana voters to register and vote on Election Day at their polling place for primary, general, municipal, school district, or special elections, effective July 1, 2019

Requires same-day registrants to complete a voter registration form, sign an affirmation that they have not voted elsewhere in the election, and provide proof of residence

Accepts three forms of proof of residence: current valid photo ID, current utility bill/bank statement/government document showing name and address, or a sworn affidavit from another registered voter in the precinct vouching for residency

Voters who register on Election Day but cannot provide required documentation must cast a provisional ballot and submit proof of residence to the county voter registration office or precinct election board before the ballot can be counted

County election boards must provide precincts with sufficient registration forms, affirmations, and affidavits; completed forms are attached to poll lists for processing by county voter registration offices
