Requires the Office of the Secretary to apply by September 1, 2026 for a separate Medicaid waiver to provide assisted living services to individuals aged 60+ who meet nursing facility level of care requirements, with a stakeholder work group to assist development

Removes Medicaid recipients from managed care after 100 consecutive days of nursing facility services (effective July 1, 2027), transitioning them to fee-for-service Medicaid

Requires waiver service recipients to review billing records and report errors or inconsistencies within 45 days; providers must furnish itemized service statements upon request (up to twice yearly)

Extends the deadline for government units to file Medicaid estate recovery claims from 120 days to 9 months after a decedent's death

Reduces required public notice period for health facility reimbursement reductions from one year to six months, and requires development of a new home health services reimbursement methodology by November 30, 2026
