Day care facilities must keep completed medical records on file for children under 10 enrolled in care and children under 16 residing at the facility, using a form provided by the Kansas Office of Early Childhood director

Health assessments may be conducted by physicians, physician assistants, advanced practice registered nurses, chiropractors, or nurses approved to perform health assessments

Medical records must include both the health assessment results and medical history obtained from the parent or legal guardian

Day care providers with temporary permits or licenses must review each child's medical history with parents or legal guardians at least once every 12 months
