Requires the dementia services coordinator to submit annual reports on the Kentucky Alzheimer's and Related Dementias State Plan implementation to the Legislative Research Commission by December 1 of each year

Expands the Alzheimer's Disease and Related Disorders Advisory Council from 15 to 16 members by adding a second caregiver representative for individuals with dementia

Changes state plan submission requirements to deliver updated plans to the Legislative Research Commission every four years (next due December 1, 2029) instead of to the Governor

Mandates the council develop and implement year-long initiatives annually, beginning December 1, that advance key areas of the state dementia plan

Requires creation of a health care provider toolkit on early detection and diagnosis of Alzheimer's by December 1, 2027, to be distributed to managed care organizations, health centers, hospitals, and medical associations
