Minimum reimbursement to out-of-network ground ambulance providers is set at either the local government's established rate or, if none exists, the lesser of 400% of Medicare reimbursement or the provider's billed charges

Insurers must reimburse out-of-network ground ambulance providers directly within 30 days of receiving a claim, and providers cannot bill patients beyond their required cost sharing

Effective date is January 1, 2027, applying to health benefit plans issued or renewed on or after that date
