Amends KRS 304.17C-085 to exclude from the definition of "covered services" any services and materials for which reimbursement would be available but for the enrollee's annual maximum benefit limitation

Prohibits participating providers from being required to offer plan-set fees for services unless those services are covered under the participating provider agreement

Requires that reimbursement paid by limited health service benefit plans be reasonable and prohibits nominal reimbursement used solely to classify services as "covered"

Makes the provisions non-waivable by contract, rendering void any contractual arrangement that conflicts with or attempts to waive these requirements

Applies to contracts issued or renewed on or after the effective date of the Act
